Friendship Amulet is a special item exclusive to Co-op Mode, that gives all players in a co-op stage a level-up for one of their equipped weapons.
Players in a co-op stage may choose the Friendship Amulet as a random choice after levelling up, starting at level 30, and it may continue to appear in subsequent level-up choices even if one of the players has previously selected it. It disappears from the level-up selection after all players reach the maximum level on all of their weapons.
Friendship Amulet is unlocked by default.

- Although the only way to obtain a Friendship Amulet during a run is through the item selection after levelling up, it falls into the same category as all pickup items in the game's code. Specifically, the class of items that may drop by breaking light sources.
- A similar instance of this is the Big Coin Bag, which is unobtainable as a pickup and can only appear as a choice after a player fills up their inventory and maxes out their items. Like the Friendship Amulet, it is categorized as a pickup item in the code.
- If one player has reached the maximum level on all their weapons but the other players have not, the Friendship Amulet will continue to appear. If chosen in this circumstance, the player with no weapons left to level up will receive a level-up for one of their equipped passives instead.
